We will verify your identity by comparing this information to our business records. We cannot respond to your request or provide you with Personal Information if we cannot verify your identity or authority to make the request and confirm the Personal Information relates to you.
You may have an authorized agent submit a request on your behalf. A parent or guardian may also submit a request on behalf of a child.
